French doctors speak about face transplant surgery

face Two years ago, French doctors performed the first human face transplant, a surgery that shocked and intrigued people around the world. The surgeons have shared intimate details about the ups and downs of the operation.

Isabelle Dinoire was severely disfigured in May 2005 by her dog. Surgeons grafted the lips, nose and chin of a brain dead woman onto Dinoire's face. Despite two early rejections, the groundbreaking surgery is considered a success.

During an emotional presentation Friday at Advocate Lutheran General Hospital, doctors revealed the painstaking plans and the grueling process involved in the transplant. They say the results are far beyond their expectations and believe it was their duty to help the woman.

"There was a big fear of rejection, and the other fear was that the patient would not accept a dead face, a face coming from the dead. In fact, that is not a dead face. The face graft is connected to nerves-- it is connected to the brain of the patient . It functions in relationship with her emotions and feelings&it really belongs to the patient," said Dr. Loren Schechter a plastic surgeon from Advocate Lutheran Gen. Hospital.

Because of patient privacy and the graphic content, cameras were not allowed in for the presentation.

Doctors say it is not a matter of if a face transplant will eventually be performed in the United States but when.

$2 million bail set for unlicensed plastic surgery

prison A couple accused of disfiguring women and practicing medicine without a license had their bail raised to $1 million each by a Santa Clara County Superior Court judge Friday during their arraignment.

Ha Nguyen, 48, and Zbigniew Makowski, 62, remain in the county jail after being arrested Wednesday on charges of illegally performing cosmetic surgery that disfigured at least one woman.

They also have been charged with child endangerment after four children were found in a home that investigators described as filthy and filled with syringes and drugs.

The citizenship status of the couple isn't known, said deputy district attorney Michael Adams, but he believed they are a flight risk. Makowski may be a Canadian citizen, Adams said, but he wasn't sure whether Nguyen, who is Vietnamese, is in the country legally.

`I think she's a genuine risk to public safety," Adams said of Nguyen, who is accused of performing cosmetic surgeries including tummy tucks and face lifts with Makowski's assistance. Adams said he did not know if Nguyen has any medical training.

Exercise, yoga give breast cancer patients a lift

Women with breast cancer who did aerobic and weight training exercises had better self esteem, and the weight lifters were more likely to complete their chemotherapy, the study of 242 women in Canada found.

And a group of mostly black and Hispanic women with early breast cancer reported improvements in quality of life after taking a gentle yoga class each week for 12 weeks.

Both studies, published in the Journal of Clinical Oncology, suggest lifestyle changes such as exercise and yoga may help women with the emotional and physical challenges of breast cancer.

In the biggest study of its kind, researchers at the University of Alberta studied women undergoing chemotherapy in 2003 through 2005.

They were divided into three groups - an exercise group, a weight training group and those getting usual care in which patients are routinely told to take it easy.

Women in both exercise groups reported improved self-esteem, physical fitness and body composition.

And women in the weight-training group had the best chemotherapy completion rate of the three groups, with 78 per cent completing the recommended chemotherapy, compared with 74.4 per cent in the aerobic group and 65.9 per cent in the usual care group.

Outpatient surgery clinics have sprouted up in droves all over the United States and the world. The number of cosmetic procedures performed in the US alone has increased by over 95% from the previous year. Although procedures like facelifts and abdominoplasties (tummy-tuck) should be considered minimally invasive surgeries, the anesthetic protocols and regimens are often overly complex and unnecessarily toxic.

Major complications involving anesthesia can range from severe postoperative nausea and vomiting (PONV) to postoperative pain and mortality. The mortality spectrum of things may be rare, but there have been many cases in which perfectly healthy cosmetic surgery patients require emergency intervention due to a severe complication involving anesthesia.

Dr. Friedberg has developed BIS monitored propofol ketamine sedation, also known as minimally invasive anesthesia (MIA), designed to maximize patient safety by minimizing the degree to which patients need to be medicated to create the illusion of general anesthesia with the lesser trespass of non-narcotic sedation. MIA eliminates PONV without the use of anti-nausea medications while providing pre-emptive pain relief that eliminates the need for postoperative narcotic medications (and their side effects).

For greater patient safety, Dr. Friedberg deems BIS monitoring a standard of care, especially for cosmetic surgery.
